If your child ever
goes off campus with us (whether for an afternoon or for a week), we MUST have
the release form to ensure we can get medical help for your child should the
need arise. It’s good for a full school
year, so go ahead and print one up, fill it out, get it notarized, and turn it
in so that your kid can be set for a year!
*adult chaperones must fill them out for trips as well*
